Patient Information

To obtain a referral for treatment, contact your primary care physician and obtain a prescription for Occupational Therapy evaluation and treatment. The prescription (or referral form) should contain the diagnosis for which you are being referred to Raven Hill Integrated Therapies, LLC, and any specific instructions your physician, physician assistant or nurse practitioner has for the therapist.

While it is not necessary to get a referral in order to be treated, insurance companies will require a physician's referral before they will pay for treatment. We do accept cash payment for treatment for those patients without insurance, as well as MasterCard and Visa, for those without insurance coverage or without a physician's referral.

Once you have a referral (or prescription) in hand, you may call for an appointment (if your physician/primary care provider has not already done so for you). Providing us with the necessary information to contact your insurance company should also be done during the same phone call.
